Job Description: Case Manager

Location: Rockaway, Queens

Schedule: Tuesday–Saturday, 8:30 AM – 4:30 PM

Employment Type: Temporary long term, possible perm opportunity if FTE opens

Hourly Rate: $30–$34/hr

Start Date: Immediate

Position Summary

The Case Manager provides on‑site case assistance, resource navigation, program intake support, and community engagement for members. This role supports the social, emotional, and practical needs of program participants while maintaining program records, facilitating groups, and offering leadership to other case managers across Queens and Manhattan.

Key Responsibilities

1. Deliver on-site case assistance including information, referral services, and member support.
2. Complete program intakes, assessments, and maintain accurate records in accordance with agency and program standards.
3. Plan and facilitate community education and recreational groups for center members.
4. Provide supervision and guidance to Queens/Manhattan case managers , supporting program consistency and quality.
5. Assist with general center operations and tasks as assigned by the supervisor.

Required Qualifications

• Master of Social Work (MSW)required.
• Experience providing direct services such as case management, counseling, or social services.
• Strong interpersonal and communication skills.
• Ability to maintain accurate documentation and meet deadlines.
• Ability to work independently and collaboratively in a community-based environment.

Preferred Skills

• Experience working with older adults or vulnerable populations.
• Group facilitation experience.
• Familiarity with community resources in Queens/NYC.
• Bilingual abilities are a plus.

Work Schedule

• 5 days/week , Tuesday–Saturday
• 8:30 AM – 4:30 PM

This is a Contract to Hire position based out of Queens, NY.

The pay range for this position is $32.00 - $38.00/hr.

This is a hybrid position in Queens,NY.
