13-Week Travel Contract | Day Shifts | 40 Hours Guaranteed

We are currently seeking an experienced Home Health Registered Nurse (RN) for a travel assignment in California. This position is ideal for clinicians who enjoy independent field-based work while providing compassionate care to patients transitioning from hospital to home.

Job Responsibilities:

Provide skilled nursing care to Home Health patients in residential settings
Perform Start of Care, Recertification, Resumption of Care, and routine nursing visits
Educate patients and families on disease management, medications, wound care, and medical equipment
Care for patients with diagnoses including CHF, COPD, wounds, wound vacs, and IV/PICC lines
Complete accurate documentation and care planning
Coordinate with interdisciplinary healthcare teams to ensure continuity of care
Maintain productivity expectations and daily visit schedules

Requirements

Active RN License
Minimum 2 years of Home Health or relevant nursing experience required
Current BLS Certification required
Valid driver’s license and active auto insurance required
HomeCare HomeBase (HCHB) experience preferred
Strong assessment, communication, and time-management skills
Ability to work independently in the field
